Tuition Adjustment Program
The Parent's Financial Statement (PFS) is your application form.
The form is quite lengthy and detailed, and may require you to consult your financial records from current and past years. Give yourself plenty of time to complete it. It consists of four parts:
Part 1 - basic information about you and about the students you support
Part 2 - a statement of your annual income and expenses
Part 3 - a statement of your assets and liabilities
Part 4 - how you managed this year's fees
Before completing the form, please be sure to read all the information here on our web site. Use the menu items under "Tuition Adjustment" in the menu to the left. If you have any questions, you can e-mail our Admissions Coordinator, Aileen Stewart, or call her at 905-881-6137 ext. 314.
Copies of the relevant Canada Revenue Agency forms (T4, T776, T2032, or T2124, or possibly others) must accompany your completed form. See the note on page 2 of the form for details.
In situations involving single parents, separated or divorced parents, or joint custody, there are special guidelines for the information needed. See the note on page 3 of the form for details. All information is kept confidential from the other parties involved, and can be sent either jointly or separately.
Please make a copy of your application form for your own records before mailing it.
